I AGREE THAT

• No one other than PRO is authorized to record any portion of the meeting and to do so without everyone's consent is illegal.

• The meeting facilitator will be recording the meeting for internal use only and you consent to record by PRO.

• This meeting is intended for registered participants only.

• This is a confidential setting and as such, do not divulge names or identities of participants or anyone mentioned.

• This is a support group, not a judgment group. We are here to support one another through the journey of living with someone living with Parkinson's.

• We participate as individuals affected by Parkinson’s. Unless otherwise stated, no one in attendance is acting in their professional capacity.
